Membership includes:

  • Monthly follow ups for continued education and support
  • Unlimited access to your provider via messaging or email
  • All follow up labs
  • Unlimited access to discounted medical grade supplements with ability to use FSA/HSA
  • Continued prescribing
  • BEST for individuals with more complex concerns that will need work up and treatment in steps

12-month membership: $199/month or $2,000 up-front fee for the year

6-month membership: $249/month or $1,250 up-front fee for the 6 months MOST POPULAR

Non-Member Option

You can have follow up as a non-member, at least every 3 months until stabilized at $349 (which does include the visit and repeat labs).

Medication checks only; no labs or other evals, at $99 for non-members.

We do not take insurance, but we do provide a “Super Bill” that can be submitted to your insurance company for reimbursement directly to you if you have out-of-network benefits

Medical Card Certifications

Guidance for patients interested in cannabis for medical support.

We will review your medical history and determine if you meet eligibility per your state regulations to obtain a medical card.

NH residents must have their initial appointment with us in person per state regulations. Afterwards, they can recertify via telehealth.

MA residents can utilize telehealth or in-person for any of their visits.

Please email a copy of your medical diagnoses from your PCP/specialist as well as a copy of the front of your driver’s license to support@faroutalternativecare.com.

If you do not have medical documents proving your diagnoses you should choose the “requiring diagnosis” option so our provider can have a more in-depth consult evaluating you for a qualifying diagnosis,
